About This Program

Botanical Sciences operates an SMS messaging program to deliver patient intake forms to new patients at the point of in-person registration. This page describes how the program works, how consent is collected, and how to opt out.
 

Program Description

When a new patient registers in person at a Botanical Sciences location, our staff will ask for a mobile phone number to which we can send a one-time text message containing a link to a secure online intake questionnaire. The patient completes this questionnaire prior to or during their first visit.
 
The phone number provided for this purpose is used solely to send the intake form link. It is not used for marketing, appointment reminders, refill notifications, surveys, promotional communications, or any other purpose. Marketing and other communications, if any, are operated under a separate program with independently captured consent.
 

How Consent Is Collected

Consent for this SMS program is collected verbally and in person at the Botanical Sciences front desk during patient registration. Before any text message is sent, the patient is informed of the following:
“We’ll send a one-time text to this number with a link to your intake form. We won’t use this number for marketing, appointment reminders, or any other purpose. Message and data rates may apply. Reply STOP to opt out, HELP for help.”
 
The patient must affirmatively consent to receive the intake message before staff initiate the send. Each consent event is logged with a timestamp, the staff member who collected it, and the version of the disclosure language presented.
 
By providing your phone number to Botanical Sciences staff at registration, you consent to receive a one-time SMS message containing a link to your patient intake form.
 

Message Frequency

This is a low-frequency, transactional program. Patients typically receive one (1) message per registration event. In rare cases, a follow-up message may be sent if the original message fails to deliver or if the intake link expires before completion. Total messages from this program are expected to be no more than two (2) per patient per registration.
 
You will not receive recurring or scheduled messages from this program.
 

Message and Data Rates

Message and data rates may apply. The cost of receiving SMS messages is determined by your mobile carrier and is not controlled by Botanical Sciences. Please consult your wireless plan for details on messaging rates.
 
Supported Carriers
This program is supported on all major U.S. wireless carriers, including AT&T, Verizon, T-Mobile, and most regional carriers. Carriers are not liable for delayed or undelivered messages.
 

How to Opt Out

You can opt out of this SMS program at any time by replying with any of the following keywords to a message from Botanical Sciences:
  • STOP
  • UNSUBSCRIBE
  • CANCEL
  • END
  • QUIT
Upon receiving any of these keywords, you will receive a one-time confirmation message acknowledging your opt-out, and no further intake messages will be sent to your number.
 
If you opt out and later wish to receive an intake link for a future registration, you may either:
  1. Reply START, JOIN, YES, or SUBSCRIBE to the most recent Botanical Sciences SMS thread to resubscribe; or
  2. Provide your phone number again, in person, at the time of your next registration.

Privacy

Phone numbers collected for this program are stored securely and are not shared with third parties for marketing purposes. The phone number is used only to deliver the intake form link via our SMS provider (Twilio Inc.), which acts as a service provider under a Business Associate Agreement.

Changes to These Terms

Botanical Sciences may update these SMS Terms from time to time. The “Last Updated” date at the top of this page reflects the most recent revision. Material changes will be communicated to active program participants where required by law.
